
Financial Policy.
Thank you for entrusting us with your behavioral care. We are dedicated to providing you with quality health care. Our payment policy is outlined below. Please read it, ask us any questions you may have, and sign in the space provided. A copy will be provided to you upon request.
Proof of insurance.
Before seeing the provider, all patients must fill out our patient information form. To submit evidence of insurance, we will need a copy of your driver's license and current valid insurance. You may be held liable for the balance of a claim if you fail to supply us with accurate insurance information in a timely way.
Co-payments and deductibles.
At the time of service, all co-payments and deductibles must be paid. This arrangement is part of your insurance company's contract. Failure to collect co-payments and deductibles may be considered fraud on our part. Please assist us in enforcing the law by paying your co-payment at each visit.
Non-covered services.
Please be advised that some – if not all – of the services you get may be uninsured or deemed unnecessary or unjustified by Medicare or other insurers. You must pay in full for these services at the time of your visit.
Telehealth Appointment.
Prior to each Telehealth visit, all co-payments, deductibles, and self-payments must be made. If a patient is uncomfortable with payments before visits, they can choose an in-person appointment instead. However, as of March 2020, due to the COVID-19 pandemic, we will not be seeing patients in the office until further notice. Furthermore, please be advised that not all insurance companies cover Telehealth sessions. If you choose a Telehealth visit, we will check with your insurance company to see whether you are covered and notify you. If you choose a telehealth appointment and are not covered, you are responsible for full payments.
Refund Policy.
Please keep in mind that NO refunds will be issued to patients who change their minds during their appointment or after successfully completing their consultation with our mental health provider, and NO refunds will be issued to patients who received and filled their medication prescription at a pharmacy.
Telehealth refunds are provided if you cancel your appointment prior to your appointment. All eligible refunds will be processed within 10 business days. If you do not get a refund, please wait at least 10 days before contacting our office.
Claims submission.
We will file your claims and assist you in any way we can to have your claims reimbursed as quickly as possible. Your insurance provider may need you to personally provide certain information. It is your obligation to follow their instructions. Please be advised that whether or not your insurance carrier settles your claim, the remainder of your claim is your responsibility. Your insurance benefit is governed by a contract between you and your insurance carrier; we are not a party to that agreement.
Coverage changes.
If your insurance changes, please contact us before your next visit so that we can make the necessary modifications to ensure you receive your full benefits. If your insurance provider does not settle your claim within 45 days, the remainder will be immediately invoiced to you.
Nonpayment.
If your account is more than 90 days overdue, you will get a letter or phone call informing you that you have 20 days to pay it in full. Unless agreed upon, partial payments will not be accepted. Please be advised that if a balance remains outstanding, your account may be sent to a collection agency, and you may be dismissed from this practice. If this happens, you will be contacted by regular and certified mail and given 30 days to locate alternative medical care.
Missed appointments.
We have a policy of charging for missed appointments that are not cancelled within 24 hours of your visit. These charges will be your responsibility and will be billed to you directly. Late cancellations will be considered as a “no-show.” Exceptions will only be made in extraordinary circumstances. Please assist us in better serving you by maintaining your regularly scheduled appointment.

Our practice is dedicated to delivering the best available care to our patients. Our prices reflect the normal and customary charges in our area.
It is unethical to deny a patient's request for treatment solely on the basis of race, sexual orientation, gender, or personal beliefs. We do, however, reserve the right to withhold service to individuals who do not follow our policies.
